All employees of Brookwood Swim and Tennis Club are required to provide the following employment documents:
 

• Form W-4 (http://www.irs.gov/pub/irs-pdf/fw4.pdf)

• Form I-9 (http://www.uscis.gov/files/form/I-9.pdf)

• Personal Identification*: U.S. Passport OR State Issued Driver's License/School ID w/ Photograph AND Social Security Card/Birth Certificate

• Life Guards Only*: Current Life Guard Certification and CPR Certification
  
  *These documents will be photocopied and the copies will be kept on file with the Swim Club.   The originals will be returned immediately.
  
  Employees cannot begin work until the required paperwork is on file.   W-4 and I-9 forms will be provided at the preseason staff meeting or they can be accessed via the above links and turned in to the manager.
